The "second" belt he started using in 1976 was originally designed for the "White Two-piece with Blue Ornaments"from 1975.

Isn't this the jumpsuit he wore at that concert where his voice was so slurred and there were lots of problems? I read somewhere that some even wanted a refund and possibly he was asked not to come back. I might have my facts screwed up here. Am I even close?

You are probably referring to the infamous College Park concerts in September 1974. If this is the case then the answer is yes. This was one of the suits. He did two shows. On September 27th he wore the "Peacock" and on September 28th the above described suit which original name was the "Blue Swirl".
a great suit and one that looked fantastic on him and shined, even though it looked simple it wasn't , it really looked great.
Sabbath I think the one from 1974 you posted is the 20th of Augst? because first 2 shows he wore the jumpsuits then he started wearing the tan 2 pieces suits. I think not 100% sure. I know he wore the Peacock first night then Rainbow, and I think even the Light Blue Pheniox if you look at the one with snake on the leg...Looks like Vegas.

Also, I've seen other guys with this jumpsuit on. Its more beautiful in person. The Blue really shines in the spotlight.

this is a great suit not filled with jewels but still catchy and unique
